Referral to Occupational Health
A Manager or Head of Department with concerns about a member of staff may wish to consult Occupational Health and/or their link HR Officer for advice prior to making a formal referral using the Referral form provided below.
The reasons for the referral should be fully discussed with the individual concerned before the form is sent to Occupational Health and a copy sent to Human Resources.
The Referral Form can be either filled in on-screen, printed off, signed and sent to Occupational Health (Room 3.109) or may be printed off and filled in by hand.
All members of staff can make direct contact with Occupational Health regarding any health concern or sickness absence.
