     Ward level data request form

As part of its effort to comply with the 1998 UK Data Protection Act, the National Centre for Social Research requires us to ask users of the 2005 BES survey data with ward-level variables to indicate explicitly that they will not use these variables to attempt to identify individual survey respondents. Please so indicate by filling the form out below which indicates your agreement to the following:

'I request that you send me a copy of the 2005 BES data with the ward identifier variables. I will use these data strictly for my personal legitimate academic research and will not use the ward variables to attempt to identify individual survey respondents. Also, I will not distribute the 2005 BES data with ward-level variables to any other persons without the explicit written permission of the 2005 BES principal investigators.'

In addition, please indicate if you want an SPSS or a STATA data file.
